REASON OR CAUSE:
1.
Overfueling of engine caused by defective or misadjusted fuel injection pump.
2.
Incorrect turbine housing (incorrect A/R ratio) or incorrect nozzle ring, small vane gap.
3.
Incorrect nozzles - orifice size larger than specified for application.
